Asrock Z690M D5 Riptide IO shield MATX
This was a problem solved for me because I bought a MB without an IO shield. Didn't know that I would probably find a Unicorn before finding this shield. So, I uploaded the picture of the rear inputs and outputs to Lightburn, added circles and squares to the right places, removed the picture, grouped all of that mess, and exported it as a SVG. Uploaded it to Tinkercad along with a blank IO shield. Combined them, then moved them around until I got them aligned, then 3D printed it. 3 iterations later, I have an IO shield that is better than not having one!
